Access to the clinical portal includes a network that brings together information from participating organizations’ (POs) clinical information systems, including electronic health records (EHRs), and provides the ability for PO’s authorized providers and support staff to securely access patient care information across the New Mexico community. Health information is exchanged electronically, privately, and securely consistent with HIPAA.
The health information exchange allows the patient record to follow the patient as they move through different points of care in the health delivery system (doctor’s office, lab, hospital, specialist, therapist).

Clinical Data Repository (CDR)
The SYNCRONYS CDR consolidates data from a variety of clinical sources to present a unified view of a single patient. Data types stored in the SYNCRONYS CDR include patient demographics, allergies, encounter history, diagnoses, medication history, immunizations, procedures, problems, lab results, pathology, radiology, and patient notes.
Enterprise Master Patient Index (EMPI)
The NextGate EMPI database verifies the unique identity of individual patients to consolidate patient records from multiple facilities under one exchange-wide universal identifier.
Privacy & Security Management
SYNCRONYS allows for secure access by authorized users. Participating providers are also required to meet the rigorous Health Insurance Portability and Accountability Act (HIPAA) regulations and Health Information Technology for Economic and Clinical Health (HITECH) Act privacy and security standards, as well as comply with applicable New Mexico requirements. SYNCRONYS offers services to assist users to comply with regulatory requirements, such as risk assessment and analysis, policy and procedure review, and development and training.
eHealth Exchange Network Gateway
eHealth Exchange Network Gateway: Participant organizations may connect to the eHealth Exchange Network The Sequoia Project through SYNCRONYS. This service creates a secure, standards-based, auditable and bi-directional connection between SYNCRONYS and the eHealth Exchange network. The network allows exchange of patient information with other HIEs and federal agencies such as the Veterans Administration, Indian Health Services, and the Social Security Administration.
Continuity of Care (CCD)/Continuity of Care Architecture (CCDA) document exchange via HIE protocols, including XDS/XDR.
The intent is to exchange as much of the US Core Data for Interoperability (USCDI) data set as possible, to enhance the data available to SYNCRONYS HIE users. The ability to exchange the data in the USCDI will be required of your certified EHR vendors.
Participants in the HIE will have access to advanced directive(s) and MOST forms through our partnership with Vynca. MOST forms can also be created within the portal.

SYNCRONYS extracts data from Orion Health’s Clinical Data Repository (CDR) and then transforms and loads it into its own on-site data warehouse. This organizes data into a schema suitable for reporting and analytic purposes. SYNCRONYS offers custom queries, extracts, reports, graphics, statistical analysis, and data exploration tools. Each organization has its own unique goals for data analytics. Analytics can be pushed to customers as needed to fit their needs. SYNCRONYS has secondary data use policies and procedures that ensure appropriate access in compliance with all state and federal laws.
Direct Secure Messaging provides point to point secure encrypted communications. Use of Direct Secure Messaging is considered by the Office of the National Coordinator to be the preferred method of sharing information during transitions of care and is therefore a measure in the Promoting Interoperability initiative.

SYNCRONYS has been authorized by the New Mexico Department of Health to provide Reportable Lab Results reporting and Syndromic Surveillance reporting to NM Department of Health for New Mexico hospitals.
COORDINATE is an optional, HIE integrated software suite that works with the traditional HIE to improve care coordination, which includes transition of care management. Also included is notification and reports for patients with a transition of care, patient prioritization and management, readmissions management, length of stay management and population insights. A feedback loop between acute and post-acute care facility and additional information from acuate and post-acute discharge plans is available, as well as creating a circle of care for the patient based on friends, family, and providers. Post-acute discharge plans, ambulatory visits, home health care and inpatient visits is also provided. This solution is customized based on the facility needs.

SYNCRONYS can provide solutions for real-time notifications of hospital admissions, discharges, emergency department (ED), or lab result activity to the physician or to a care management team. SYNCRONYS will work closely with participants to enable this functionality.The CMS Interoperability and Patient Access rule, also known as CMS-9115-F, updates the existing conditions of participation to require hospitals—including psychiatric hospitals and critical access hospitals—to send electronic patient event notifications of a patient’s admission, discharge, and/or transfer (ADT) to other healthcare facilities or to community providers or practitioners.
