Clinical Portal – Access to the clinical portal includes a network that brings together information from participating organizations’ clinical information systems, including electronic health records, and provides the ability for participating organizations’ authorized providers and support staff to securely access patient care information across the New Mexico community. Health information is exchanged electronically, privately, and securely consistent with HIPAA.
Advanced Directives – Participants in the HIE will have access to advanced directives and MOST forms through our partnership with Vynca. MOST forms can also be created within the portal.
Diagnostic Quality Images – Integrated links to diagnostic quality images in radiology reports. Tools to transfer images. Viewing images is available to HIE Core users.
Patient Alerts & Event Notifications – SYNCRONYS can provide solutions for real-time notifications of hospital admissions, discharges, emergency department, or lab result activity to the physician or to a care management team.
MCOs & Payers – SYNCRONYS supports and contracts with all MCOs throughout New Mexico, including those awarded under Turquoise Care, the New Mexico Medicaid Managed Care program that began July 1, 2024. MCOs and Payers include Blue Cross and Blue Shield of New Mexico, Molina Healthcare of New Mexico, Presbyterian Health Plan, and UnitedHealthcare Community Plan, as well as Presbyterian Health Plan for Medicare Advantage Marketplace.

To support population health, SYNCRONYS is used to analyze aggregated health data across a population, enabling both health care providers and public health officials with the New Mexico Department of Health, to identify trends, target interventions, and monitor disease outbreaks.
Priority programs and initiatives SYNCRONYS currently supports in New Mexico are behavioral health, prenatal health care and the treatment of Hepatitis C.
The benefit SYNCRONYS provides to the medical community and patients through our solutions include improved quality of care, cost reduction and efficiency. SYNCRONYS provides a complete picture of a patient’s health history allowing for a more accurate diagnoses to be made, and potential health risks identified. The use of the HIE is shown to reduce redundant tests which are often paid out of pocket by the patient while eliminating the time and stress of potentially unnecessary procedures. The use of the HIE improves the efficiency of a medical practice as records from outside of their facility are obtained through the HIE central repository, no longer requiring additional time and personnel to fax records, or needing the patient to provide them.
Dedicated Team – Quality data is crucial for patient care, decision-making, outcomes, and more. SYNCRONYS has a dedicated Data and Analytics team who work continuously to build and expand the data quality of the HIE and improve the usability of the data.
Validated Data Streams – In 2024 the National Committee for Quality Assurance (NCQA) validated data streams from Presbyterian Medical Services, Presbyterian Healthcare Services, and University of New Mexico Health, allowing SYNCRONYS to offer them as standard supplemental data to Managed Care Organizations (MCOs). SYNCRONYS will also participate in the July DAV cohort in 2025.
Health Equity Map – This application allows users to analyze global and attributed populations. It includes super filters to group related diagnosis codes for filtering, and incorporates over 150 publicly available data sets, which can be overlaid and combined with patient-specific data for a comprehensive picture of health and disparities in any region. The HEM also includes the following use cases to help users make the most of the data: Population Health, Access to Service, Children and Adolescent Health, Maternal Health, Behavioral Health, Native American Health, Quality Metrics, and MCO Comparison.
Behavioral Health Dashboard – The HIE has collected and aggregated data to create actionable behavioral health dashboards, allowing providers to make targeted interventions and improve patient outcomes.
Public Health Reporting – SYNCRONYS has been authorized by the New Mexico Department of Health to provide Lab Results reporting and Syndromic Surveillance reporting to the New Mexico Department of Health for New Mexico Hospitals.
Syndromic Reporting – The National Syndromic Surveillance Program is used by the CDC to anonymously track public health via diagnostic information collected from Emergency Departments and is being expanded to include inpatient and outpatient visits from both hospital and non-hospital entities. SYNCRONYS collects and maintains a database of this data for the New Mexico Department of Health and forwards an anonymized dataset to the CDC.
Disease Reporting – The New Mexico Electronic Disease Surveillance System is used by the New Mexico Department of Health to collect laboratory-reported diseased such as COVID, Flu, Salmonella, and Measles. SYNCRONYS collects and maintains a database of this data for NMDOH and forwards specific portions of it to them for statistical analysis and contact tracing.
