Substance Use Disorder: Hospitals, Substance Use Disorder treatment providers, and ambulatory clinics will all be connected to the platform and able to collaborate with the goal of improving care coordination and patient care for those affected by SUD through:

  • Hand-off Notification to MAT clinics and other treating BH providers.
  • Establish an ephemeral patient-provider relationship when a patient handoff occurs where a provider does not have a pre-existing patient attribution.
  • Infants with NAS are tracked across the network.
  • Overdose Notifications.

Mental Health Management: Surface awareness enables collaboration for patients with mental health needs across both acute and ambulatory settings allowing for improved outcomes. Mental Health Management tools include:

  • History of recent suicide and self-harm diagnosis.
  • History of recent mental / behavioral health diagnosis.
  • Organization has option to “tag” patients (manually or automated).
  • Identification of crisis plan triggers.
  • Mental health notifications are sent to providers at the point of care, in their existing workflows, with important and relevant patient information.
  • Real time notifications, via email or text, provide collaboration between internal teams.

Emergency Department Optimization: Providing relevant patient-specific alerts and information to hospitals can drive improved decision making in the ER. Emergency Department Optimization opportunities include ensuring the ER staff has access to critical information such as:

  • Outpatient Diagnoses for ER view.
  • Vaccines (i.e. Flu or Covid).
  • Opioid Medication Fill – Opioids from multiple providers, Use of opioids at high usage.
  • Medication Fill Data for key chronic conditions.
  • Results of key tests.

Optimizing HEP-C Analytics for Population Health:
Together with Rhodes, we will deliver insights through the Orion platform for enhanced HIE value which includes addressing the prevalence of the HEP-C virus in New Mexico, by providing the patient’s HEP-C status and comprehensive laboratory results in the HIE.
