SYNCRONYS has partnered with PointClickCare to integrate real-time Admit, Discharge, and Transfer (ADT) data from 41 New Mexico Skilled Nursing Facilities into our clinical portal. This collaboration enhances care coordination, allowing providers to track patient statuses instantly and improve patient outcomes across the continuum of care. Having access to this data within the clinical portal allows for improved care coordination, especially for patients being transferred between an Emergency Department and a Skilled Nursing Facility. This data is also used to populate SYNCRONYS’ SNF Summary Insights and Daily Admission Extracts, allowing providers and care teams to keep up with the status of their patients in real-time.
It is crucial that providers have access to accurate, timely, and secure data. In addition to improved care coordination, access to a patient’s longitudinal record reduces reliance on patient self-reporting, streamlines testing and treatment planning, expedites medication management, and reduces errors and duplication.
