SYNCRONYS and Patient Centered Data Home (PCDH)
Albuquerque, NM – October , 2022 – SYNCRONYS, New Mexico’s designated health information exchange (HIE), is pleased to announce it is receiving data from Patient Center Data Home (PCDH) participants in the Civitas Western Region as of Monday October 24, 2022, via the SYNCRONYS clinical portal.
PCDH enables the exchange of patient information across HIE organizations in other states. Based upon your zip code an individual is assigned a “home HIE”. With the activation of PCDH if a New Mexican receives healthcare in a state that is part of the Civitas Western Region, SYNCRONYS as the “home HIE” will receive the alert. This trigger alert enables the SYNCRONYS to store this information and to share the relevant patient information to a healthcare provider in New Mexico to coordinate better patient care.
This newest feature within SYNCRONYS enables the sharing of critical patient data related to care received outside of the patient’s home state of New Mexico. PCDH enriches the data contained within SYNCRONYS and enhances healthcare coordination and management across disparate health care systems and the health care community.
The Civitas Western Region includes health information exchanges in Alaska, Arizona, California, Colorado, Hawaii, Idaho, Nebraska, New Mexico, Nevada, North Dakota, Oregon, Utah, and Wyoming.
